Before your appointment

Please allow up to 2 hours for your appointment especially if you need a dilated eye exam.
You may want to bring someone with you to drive you home.

Referrals

Some insurance carriers require you to have a referral prior to your eye exam. Please contact your insurance company prior to your visit for more information.

  • We do not accept vision plan at this time.

Please bring your insurance card(s) and a valid photo ID with you at your first appointment. You will be asked to pay for all non-covered services at the time of the visit.

We accept cash, Visa, Mastercard, American Express, and personal checks for payment. If your personal check cannot be processed because of insufficient funds in your account, you will be charged $30.

Medical History

Please bring your eyeglasses as well as a list of all current medications that you are taking. If this is your first visit to our practice, please bring any previous medical records and test results. You may want to print and fill out the form below to save time.

Registration

Please arrive 15 minutes prior to your appointment time to complete the registration process.

Cancellation / Rescheduling

Please call 703-364-5400 if you need to cancel or reschedule your appointment. We would appreciate at least a 24-hour advance notice for any appointment cancellation. If an appointment is cancelled within 24 hours notice, there will be a $50 fine.
